    Team Manager, Transitions

    Sanctuary Social Care is currently looking for an experienced and dedicated social worker to work full time as a team manager within a transitions team based in Wirral. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

    The pay rate for this locum team manager job is to be confirmed and is an initial 3 month contract.

    Responsibilities of the team manager:
    - Providing effective management support and individual supervision and guidance to a team of social workers, and health professionals
    - Ensuring that the work of the team complies with statutory duties and requirements
    - Being responsible for promotion of and access to the transition service
    - Ensuring representation of the CLDS (Community Learning Disability Service) at case conferences and negotiations with other agencies

    Requirements of the team manager:
    - CQSW, Dip SW or other HCPC recognised social work qualification.
    - A current certificate of registration with the HCPC will be a condition of appointment
    - Experience of supervising professionally qualified staff and an understanding of the statutory responsibilities of a local authority and provider health services trust
    - Experience of working in a multi-disciplinary setting
