    Description

    Sanctuary Social Care is currently looking for an experienced and dedicated social worker to work full-time within a community mental health team in Bromley. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

    The pay rate for this locum social worker job is £28 per hour (LTD Company Equivalent) and is an initial 3-month contract.

    Responsibilities of the social worker:
    - Provide an assessment and care management service to individuals, their carers and other agencies.
    - Ensure the provision of an assessment, care co-ordination and care management service under CPA for service users and carers, and in collaboration with professional colleagues and representatives of voluntary and private agencies, and where appropriate, to undertake the role of Care co-ordinator
    - Monitor, review and evaluate care plans in order to ensure that they are meeting the needs of service users.
    - Hold a small caseload concentrating on those with complex care needs.
    - Review the needs of individual service users and their carers and to undertake Carer's Assessments where appropriate.
    - Identify and report unmet needs and short-falls in the construction of individual care plans.
    - Maintain client records in accordance with Oxleas NHS / Bromley Council Policy.
    - Respond to and where possible resolve complaints within the Oxleas NHS / Bromley Council Complains procedure.

    Requirements of the social worker:
    - Degree or equivalent in social work
    - Current HCPC registration
    - Significant senior level frontline experience within adult services
    - Experience of working with people with complex, acute and/or enduring mental health needs which may be challenging at times.
    - Experience of working in collaboration with other mental health workers, both in the community and hospital setting.
    - Car driver
