Sanctuary Social Care is currently looking for an experienced and dedicated social worker to work full time within an intermediate care team based in Hertfordshire. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

The pay rate for this locum social worker job is £26 per hour (LTD Company Equivalent) and is an initial 12-week contract.

Responsibilities of the social worker:
- To manage the care of people who require support on discharge from a hospital.
- You will need to have the skills which enable you to lead a team effectively to ensure flow and capacity is maintained within the community rehab services.
- To improve outcomes for service users, the Integrated Discharge Service supports a 7-day Hospital Discharge service.
- To provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.

Requirements of the social worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adult services
