    Are you interesting in developing your social work career?

    Sanctuary Social Care is currently looking for an experienced and dedicated social worker to work full time within a hospital discharge team based in Somerset. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

    The pay rate for this locum social worker job is £28.22 per hour (LTD Company Equivalent) and is an initial 3-month contract.

    You will be working in a multi-disciplinary, integrated Health and Social Care team and have a wide remit to work with disabled adults and support discharge to assess provided by out health colleagues within the community. The team provides the enablement service, unique to Somerset, which assists people who have recently moved into care homes to regain or maintain their indolence within the community.

    Requirements of the social worker:
    - Degree or equivalent in social work
    - Current HCPC registration
    - Significant senior level frontline experience within adult services
