Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work within a Continuing Healthcare Team based in Islington. The ideal candidate will possess highly developed specialist knowledge of Social Work practice in Adult Services and be registered with the HCPC.

The pay rate for this locum Social Worker job is £32 per hour (LTD Company Equivalent) and is an initial 3-month contract.

Requirements of the Social Worker:
- Degree or equivalent in Social Work.
- Current HCPC registration.
- Significant frontline experience within Adult Services.
- The Social Worker will need to have 6 years post qualifying experience
- The social worker will need to have worked within a MDT and Health team and have an understanding of the CHC process.
- Understand the DST process in CHC and have had substantial experience of completing CHC assessments.
- Strength working with patients and families that are facing chronic ill health and bereavements and be able to work with in the multidisciplinary team to carry out accurate assessments.
- Good knowledge of health conditions that impact on elderly patients
