Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work within a Community Physical Disabilities and Mental Health Team based in Cheshire. The ideal candidate will possess highly developed specialist knowledge of Social Work practice in Adult Services and be registered with the HCPC.

The pay rate for this locum Social Worker job is £21.72 per hour (LTD Company Equivalent) and is an initial 3-month contract.

Requirements of the Social Worker:
- Degree or equivalent in Social Work.
- Current HCPC registration.
- Significant frontline experience within Adult Services.

Duties:
- Working with all adults with a physical disability and mild to moderate mental health issues including dementia, in Chester and surrounding areas.
- The work is both patch based and integrated with health partners requiring the team to work flexibly in different locations.
- To assess vulnerable people in the community for eligible needs under the Care Act and to assess Capacity, therefore a good working knowledge of the Care Act and Mental Capacity Act is essential. there will be a need to do duty as part of the team rota.
