    Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work within a Community Adults Health Services based in Wandsworth. The ideal candidate will possess highly developed specialist knowledge of
    Social Work practice in Adult Services and be registered with the HCPC.

    The pay rate for this locum Social Worker job is £28 per hour (LTD Company Equivalent).

    Requirements of the Social Worker:
    - Degree or equivalent in Social Work.
    - Current HCPC registration.
    - Significant frontline experience within Adult Services
    - Working across two offices during the working week, workers must be able to travel to both locations.

    - The aim of this service is to build positive working relationships between Community Adult Health Services (CAHS) and Wandsworth Department of Adult Social Services (DASS) staff in order to provide a timely, joined up approach to health and social care, enabling holistic care delivery for patients and carers who would benefit most from this integrated model of care.
    - The Social Workers and Social Care Assessors will fulfil statutory functions as set out by the Care Act 2014 and DASS policies and procedures.
    - The Social Workers and Social Care Assessors will contact the appropriate identified services directly on behalf of the GP and, once a service need has been identified, they will ensure that the referring GP receives prompt confirmation of action that has been taken and the activation of services.
