Social Worker, Adults Assessment

Sanctuary Social Care is currently looking for an experienced and dedicated social worker to work full time within an adults assessment team based in Lancashire. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

The pay rate for this locum social worker job is £20.09 (LTD Company Equivalent) per hour and is an initial 3 month contract.

Responsibilities of the social worker:
- Providing high quality, responsive and safe assessment, support planning, brokerage and care planning services to meet the needs of adult service users and their family/carers and promote an ethos of positive partnerships, within specified allocated resources and in line with national and local standards and requirements.
- Delivering personalised services that promote the independence and choice of service users and carers, responds
to needs promptly and effectively, and positively manage risk.
- Providing a social work service, including assessment, support planning, brokerage, care planning and review activity, and discharge activity to an allocated caseload.
- Providing cover and participate in team duty arrangements on a rota basis in accordance with management instructions.

Requirements of the social worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adult services
