Sanctuary Social Care is currently looking for an experienced and dedicated social care assessor to work full time within a mental health team based in Wandsworth. The ideal candidate will possess highly developed specialist knowledge of social care assessor practice in adult services.

The pay rate for this locum social worker job is £12.75 (LTD Company Equivalent) per hour and is an initial 1-month contract.

The purpose of the role is to assess or review the social care needs of adults with less complex needs, where screening does not trigger the need for an assessment by a Social Worker or Occupational Therapist.
The Social Care Assessor will also work together with adults and their carers/ family to develop Care and Support Plans, to meet eligible, unmet needs and help them achieve the outcomes that matter to them in their life.

- To maintain accurate and timely case records, complete statutory reports and to share information appropriately.
- To work together with Health colleagues and other partners such as voluntary services and private providers to ensure joined-up support for adults and carers.
- To participate in supervision, training and team meetings

Requirements of the social care assessor:
- A social care qualification
- Experience of working with clients/ service users, preferably in social care
- Ability to communicate clearly and concisely; verbally and in writing, using appropriate IT.
