    Description

    Sanctuary Executive is currently looking for an experienced and dedicated service manager to fill a full-time role in Hillingdon.

    The pay rate for this locum service manager job is £350 (LTD Company Equivalent) per day and is an initial 3-month contract.

    The post holder will ensure the provision of high-quality care in the community, that all community/resident facing services are rated through the regulator as 'Good' and that the ongoing development and transformation of services keep pace with the changing statutory regulation and meet the recommendations of government best practice.

    Duties:
    - Management & performance of The Reablement/floating support team, 4 Extra Care Housing & care support units & day care for adults with LD & Older People with dementia.
    - Ensuring agreed team plans and performance targets are delivered and that a culture of 'putting our residents first' is maintained.
    - To ensure the provision of high-quality care in the community, that all community/resident facing services are rated through the regulator as 'Good' and that the ongoing development and transformation of services keep pace with the changing statutory regulation and meet the recommendations of government best practice.
    - To lead on the promotion of 'Discharge to Assess', ensure that there is capacity in the system to manage hospital discharge and ensure that they engage with and fully understand the Health integration models.
    - Understand, contribute and share with the Assistant Director the work of the ACP and other partner organisations and report relevant information in relation to BCF.
    - Be the project lead for the social care delivery in the extra care housing schemes including the 2 new 140 flats, ensuring that the care delivery and housing management is of the highest quality and achieve the appropriate outcomes for residents.
    - Be the project lead to the transfer of the Dementia Resource Service and develop the health and mental health focus for integration.
    - Be actively involved in the contract supervision of the care and support within extra care.

    Requirements of the service manager:
    - Experience of managing Social work services, specifically in planning/provision of Adult services.
    - Able to manage people to high-performance level, able to manage large budgets
