Senior Practitioner Lead, Transitions Team

Sanctuary Social Care is currently looking for an experienced and dedicated senior practitioner lead to work within a transitions team in Ellesmere Port. The ideal candidate will possess highly developed specialist knowledge of social work practice in children's and adult services and be registered with the HCPC.

The pay rate for this locum senior practitioner lead job is £24.58 (LTD Company Equivalent) per hour and is an initial 3 month contract.

Responsibilities of the senior practitioner lead:
- Providing professional expertise and support to children's social workers and other professionals working with children in need with complex needs and disabilities
- Managing the transition processes for children with disability between children services and adult services
- Ensuring that interventions are evidenced based and of high quality drawing on national best practice and research

Requirements of the senior practitioner lead
- Degree or equivalent in social work
- Current HCPC registration
- Recent experience within children's and adult services
