Sanctuary Social Care currently has a Locum vacancy based in Lancashire for a Hospital Discharge Co-Ordinator to work full time within adult services. The pay rate will be £21.65 (LTD company equivalent).

The main purpose of this role is to work with people with adult social care needs in a fast paced environment promoting independence, choice and control whilst demonstrating compassion and empathy.

Responsibilities of the Hospital Discharge Co-Ordinator:
- Managing expectations whilst treating people fairly and with respect
- Supervison and management of staff
- Being customer focussed whilst making arrangements for hospital discharges on the Home First Pathway
- Working with others to resolve conflict and achieve best outcomes
- Responding to safeguarding concerns
- Applying equality principles in practice

Requirements of the Hospital Discharge Co-Ordinator:
- Degree or equivalent in social work
- Experience within adults services
