    Description

    Sanctuary Social Care is currently looking for a Care Co-Ordinator to work full time in Hackney.

    The pay rate for this locum care co-ordinator job is £20.48 per hour (LTD Company Equivalent) and is an initial 3-month contract.

    The post holder will be an integral part of the Integrated Independence Team working with a range of health and social care professionals within the team to develop individualised care packages and rehabilitative care plans for service users, to enable daily living within a person's own home.

    Responsibilities of the care co-ordinator:
    - Ensuring high levels of care and support are implemented effectively by the team, focussing on rehabilitation, promoting independence and ensuring the individuals' safety and wellbeing.
    - Must work in a way that is flexible and able to adapt to the changes and competing demands of the service.
    - To act as a front line point of contact for the service, communicating with service users, carers, relatives, and other professionals.
    - Must Respond to daily changes in demand and make decisions on service provision that will impact on staff and service users.
