Sanctuary Social Care is currently looking for an experienced and dedicated Band 6 Social Worker to work full-time within an Older Adults CMHT Team based in Guernsey. The ideal candidate will possess highly developed specialist knowledge of Social Work Practice in Adult Services and be registered with the HCPC.

The salary for this Permanent Social Worker role is £31,531 - £42,715 per annum

Job Summary:
The main purpose of the post will be to provide a social work service to older people, their carers and family during times of change and uncertainty associated with ageing, disability, loss and grief. The post holder will be based in the Older Adults Community Mental Health Team. They will provide social care perspective in all care & professional settings; the post holder will act in an advisory capacity to members of the multidisciplinary team on social care issues & perspective relating to older adults, assisting with the mentorship of junior staff and students as appropriate.

Job Description
- To provide a social work service to older adults living in the Bailiwick of Guernsey. To carry out assessment using the agreed standardised format. To participate in co-ordinating services for people wishing to remain within their own homes. To offer support to people who may be moving into care.
- To work in a flexible way, adapting with the service to meet the changing needs of service users from a wide range of social and cultural backgrounds.
- To form relationships with service users to formulate an accurate assessment of complex psychosocial needs of service users, their families and service providers and assess for services in line with local policy.
- To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.
- To provide complex psychosocial support for service users with terminal illness/life threatening illnesses and their families, advocating for service users, carers, and families around complex emotional & practical needs.
- To provide specialist social work advice & perspective within the Older Adults Community Mental Health Team.

Requirements of the Team Leader;
- Degree or equivalent in Social Work
- Current HCPC registration
- Significant senior level frontline experience within Adult Services
- 2 years adults social care
