Advanced Practitioner, Continuing Health Care

Sanctuary Social Care is currently looking for an experienced and dedicated advanced practitioner to work full time within a continuing health care team based in Croydon. The ideal candidate will possess highly developed specialist knowledge of social work practice in adult services and be registered with the HCPC.

The pay rate for this locum advanced practitioner job is £31.15 per hour (LTD Company Equivalent) and is an initial 3 month contract.

Responsibilities of the advanced practitioner:
- Ensuring the delivery of appropriate services to children and their families, according to their assessed needs.
- Delivering good outcomes for children through taking referrals , completing timely assessments and implementing effective service plans that safeguard vulnerable children
- Leading on safeguarding investigations in the most complex cases and where there are multiple incidents
- Holding a caseload of complex cases particularly those where abuse or neglect is suspected

Requirements of the advanced practitioner:
- Degree or equivalent in social work
- Current HCPC registration
